As you age, don’t ignore changes in your vision and overall eye health. Human vision changes during every stage of life, from infancy through old age, so it’s important to have a familiarity with the most common types of changes so you know what to expect and when you should be concerned. If you have children, for example, make sure you understand that their ability to focus, see color, judge depth and remember what they see will develop throughout childhood, which means early eye exams should be focused solely on ruling out potential vision problems or abnormal eyesight.

Adulthood often brings unwelcome vision changes that are usually side effects of presbyopia (a natural hardening of the eye lens that makes it difficult to clearly focus on nearby objects). The most common complaints are slight blurring and the inability to read without a lot of light, but most of these issues can be treated by making simple lifestyle changes like wearing reading glasses.

Vision changes become the most problematic during senior years. Here are some common eye problems to watch for in your old age:

Cataracts
Approximately half of all Americans over the age of 65 develop cataracts, or a clouding of the eye lens. Cataracts can lead to blindness if left untreated, but they can usually be removed through surgery.

Macular degeneration
Also known as AMD, macular degeneration is the leading cause of blindness in American senior citizens. There’s no cure for the disorder, but early diagnosis and treatment can slow or stabilize its progression.

Glaucoma
A group of diseases caused by high pressure within the eye, glaucoma can damage eye nerves, cause blind spots or tunnel vision and, if left untreated, lead to irreversible blindness. Having an annual eye exam is the best way to detect glaucoma before it becomes untreatable with medication, surgery or laser therapy.

Eye floaters
These microscopic fibers reside in eye fluid and appear as tiny shadows, spots or flecks in your vision. Most eye floaters are harmless and don’t need to be treated, but if your floaters are accompanied by flashes of light or loss of vision, you could have a torn retina.

Q: Is there a way to keep my eyesight from getting worse?

A: Yes, there are several ways to keep your eyes healthy throughout your life. For example, we can implement strategies to protect against dry eye, macular degeneration and other diseases. Nutritional health and exercise are also important for avoiding those types of issues.

Q: Can I wear contacts if I have astigmatism?

A: Some optometrists say no, but when there’s enough astigmatism it should be corrected to provide optimal vision. If you come in for an eye exam, we’ll come up with a plan for your specific situation.

Q: Are over-the-counter eye drops safe to use?

A: It depends. Some people use eye drops to treat red eye, but that may be a sign of something more serious such as an infection or even glaucoma. Overusing eye drops can also be a problem, because they contain preservatives that may cause eye irritation. Most over-the-counter eye drops are fine for occasionally treating red eye, but you should see a doctor if you’re regularly experiencing red eye or other eye problems.

Q: Should I be concerned with little spots in my eye that move around?

A: Those spots are typically minor consequences of aging, but they could also be early signs of retinal detachment, so you should get an eye exam as soon as possible.

Q: Is there anything I can do to reduce eyestrain while working on my computer?

A: If you find you’re experiencing eye strain, headaches or fatigue while working on a computer, having a specialty pair of glasses that are optimized for computer work can help. People who work on computers tend to not blink as much as they should, so being conscious about blinking more often and using artificial tears sparingly can help. Also, taking breaks from work can be very beneficial. Try the ‘20/20/20 rule’—every 20 minutes, take a 20 second break and look 20 feet away.

Dr. Thomas Aller: A Career in Focus

SAN BRUNO — When he was in college, Dr. Thomas Aller first considered a career in animal research, but he eventually shifted his focus toward a more human-centered vocation. “I was studying animal behavior at UC Berkeley—I figured I was going to teach Koko the gorilla to write Shakespeare or something useful like that,” he laughs. “However, I ultimately concluded that a life of doing research wasn’t for me, and I went into optometry instead.”

Today, after more than 31 years practicing in the optometry field, Dr. Aller says his favorite part of his job is, ironically, research. “I’ve spent most of my career researching the preventative treatment of nearsightedness in young children, which has become a worldwide epidemic in recent years. It’s also been long regarded as a strictly genetic and incurable condition, which I took as a challenge.”

Originally from Berkeley, Dr. Aller now resides in the neighboring town of Albany with his wife, Virginia. “This is where I grew up, so it just feels like home,” he says. “Not only are the people and weather great, we also have good schools and a lot of good restaurants, so it’s an ideal place to live and raise a family.”

Outside of work, Dr. Aller engages in a variety of physical and cerebral activities. “I play handball competitively and enjoy skiing and hiking as well,” he says. “I’m also an inventor, and I’m always working on at least one project. It’s a pretty arduous process to take an idea from conception to being provable and then actually getting it patented and developed, but it’s something I really enjoy.” When he’s not hitting a ball or tinkering in his lab, Dr. Aller keeps up with his and Virginia’s three grown children: Kimberly, Brian and Theresa.

In his life and career, Dr. Aller espouses the importance of persevering in the face of adversity. “My work in curing childhood nearsightedness has been met with a fair amount of resistance because it goes against the previously held notion that it’s incurable,” he explains. “Any time you challenge a conventional view, it goes through cycles of denial and even hostility before finally reaching the point of acceptance. I’ve seen been plenty of the former, but it’s also encouraging to see more and more eye doctors starting to treat nearsightedness as a preventable condition.”

When asked the first thing he’d do if could retire tomorrow, Dr. Aller says he’d spend more time inventing. “I’d probably take out my Top Ten list of invention ideas and get serious about working on them. A colleague of mine recently retired from optometry and is now running an ‘invention incubator,’ which sounds like something I’d like to be a part of.”

  • Innovative Methods for Treating Childhood Nearsightedness

    SAN BRUNO — It’s been long believed (and still is today) that once a child is diagnosed with nearsightedness, nothing can be done to stop the subsequent regression. However, over the last decade, researchers have discovered that there are indeed ways to treat and potentially cure childhood myopia.

    One method utilizes bifocal lenses that are designed to decelerate regression of a child’s vision. As demonstrated by multiple studies performed worldwide, these special lenses have been shown to slow down nearsightedness in young children by 50 to 90 percent. This method is particularly effective for certain types of children, which can be determined during an eye exam.

    Another method is called orthokeratology: an exciting technology that utilizes corrective contact lenses. Worn during sleep, the lenses temporarily reshape the cornea and help reduce refractive errors. Studies have shown this method to slow down nearsightedness by as much as 50 percent. Besides its effectiveness, the primary benefit of this method is that the lenses can be removed in the morning so they don’t interfere with the child’s daily activities.

    A third treatment method utilizes an eye drop called atropine, which dilates the pupil and relaxes the eyes’ focusing mechanism. Researchers have found that using very low doses of atropine can be effective in diminishing the symptoms of nearsightedness with minimal side effects.

    SAN BRUNO — Host, Sarah Rutan: If you or your children suffer from Myopia, or nearsightedness, it’s important to know your options so that you can keep your eyes from getting worse. Today we’re in San Bruno with Diamond Certified Expert Contributor Dr. Thomas Aller to learn more.

    Diamond Certified Expert Contributor, Dr. Thomas Aller: Previously it was thought that with nearsightedness, or myopia, once a child gets nearsighted they just get worse and worse. There’s nothing you can do about it, and that is how nearsightedness is treated by most doctors all around the world.

    There’s been a revolution lately over the last five to 10 years certainly, and we found that there are, indeed, things that can be done to keep eyes from getting worse and worse. There are four different ways that this can be done with all with good research behind it.

    One technique is to use certain types of eyeglasses, bifocals. They’re especially effective for certain types of kids, but that can be determined during an eye exam. The research that I’ve done has to do with bifocal contact lenses. Bifocal contact lenses slow down nearsightedness by 50 to 90% as been shown in multiple studies all around the world.

    Then there’s orthokeratology, which is an exciting technology where you wear a contact lens while you’re sleeping. You take it off in the morning and you can see without them. So, kids really like that. They can swim and do sports and don’t have to worry about anything. But what’s really interesting about that technique is kids that wear that type of lens when they take them off, their vision doesn’t worsen as quickly also. So, that’s been shown to slow down nearsightedness by 50%. So, in the same range as bifocal contacts, although sometimes one works better than the other.

    And there is a treatment using an eye drop called atropine. I don’t use that that often. It’s being done very frequently in Asia. What’s really fascinating about atropine, which is an eye drop that dilates the pupil, is they found that if you use super-low doses, it actually works very well and has very low side effect, but I don’t use it that often. But there are ways to stop kids’ eyes from getting worse and worse.
